It’s honorable to be invited to Chularat IMC group. Please let me share my experience in hand and microsurgery. My first case was a young male who involve in a heat press injury.
There was third degree burn over dorsum of his right hand and the ulnar three fingers.

หลังจากผ่าตัดเนื้อเยื่อที่ตายออก
After debridement , the thumb was loss and 3rd. degree skin loss over the dorsal aspect of the middle, ring and little finger including the little finger tip.
คนไข้สูญเสียนิ้วหัวแม่มือเนื่องจากความร้อน
Thumb reconstruction was performed by great toe wrape around technique.
จำเป็นต้องรับการผ่าตัดสร้างนิ้วหัวแม่มือจากนิ้วเท้า
Thumb reconstruction was performed by great toe wrape around technique.
And STSG. to covered the dorsal finger wound.
โชคร้าย เนื้อเยื่อที่ถูกความร้อนตายเพิ่ม ทำให้ผิวหนังที่ปลูกถ่ายไว้ไม่ติด
Unfortunately there was progressive necrosis of the soft tissue of the 3 fingers. Follow by loss of the skin graft.
จำเป็นต้องผ่าตัดสร้างผิวหนังให้อีกครั้ง
New soft tissue reconstruction by a large supercharged posterior interosseous artery fasciocutaneous flap from the same side.
I connected the digital artery of the ring finger to the stump of posterior interosseous artery and placed it over dorsum of the ring finger, in oder to plan a single stage fingers separation. Digital web space were designed and reconstructed at the of flap inset.
6 เดือนหลังผ่าตัดครั้งสุดท้าย
After 6 week when tissue swelling disappear, all the finger were separated in the same setting
